Do I have to call myself an alcoholic, or promise never to drink again?

No. The work is about whether alcohol is giving you more than it's taking, and putting a real decision back in your hands.

I've stopped before and it didn't last. Why would this be different?

Because willpower runs out. We work on why you drink and give you a question you can run in the moment, until the change is automatic.

My health is already paying for it. Is it too late?

For most people, no - though if you're physically dependent, see a doctor for a medical detox first. That comes before any coaching.
